New Student Health Forms

Health Forms for Admission to School:

Requirements of the NJ Department of Education and Florham Park Board of Education mandate all health forms be completed and returned to the Health Office prior to your child's entrance to school.

Forms are available on the Brooklake Elementary School Home page "Registration" site or under "Important Forms."

New students are required to have a recent physical exam (completed within the last 365 days prior to admission), updated immunization schedule, and all other completed health office forms prior to entrance to BKL..



Please complete all of the following forms and submit to the school nurse for review:


  1. New Student Physical Examination - completed within the last 365 days prior to admission

  2. Updated Vaccination Records

      • If your child is on a catch-up schedule, they must have a minimum of 1 dose for all required immunizations before starting school.

      • If your child has recently been vaccinated for COVID-19, proof can be forwarded to the school nurse.

  3. Mantoux Form - required for students born in a country with a high incidence of tuberculosis

  4. Medical Concerns - Please contact the school nurse if your child has any additional health issues (asthma, allergies, diabetes, seizures, etc.) so the appropriate forms can be provided to you.


Important Notice: Due to COVID-19 related international travel restrictions, any unvaccinated new students traveling from international countries or territories are required to quarantine for 10 complete days or 7 complete days with a molecular test (PCR or NAAT) collected on days 5, 6, or 7 of exposure.
