One form per child.
You will receive an email with instructions to create a Powerschool Parent Portal account, and fill out Enrollment Forms.
Once the online portion is complete, continue to the next step.

#2 Documents / Documentos
Make an appointment to drop off documents at Overbook by calling the main office at 203-468-3305.
The Health Form can be faxed directly to the school by your child's pediatrician: FAX 203-468-3306
The following documents are required:

Section 1: Child’s Documentation
CT Early Childhood Health Assessment Record and Immunization record
Flu shot (required in the fall by January 1, 2025)
Custody or guardianship papers, parenting plan, and/or protection orders (if applicable)
Disability or special needs documentation (Example: Birth to Three, IEP, 504 Plan, speech or OT services) (if applicable)
Section 2: Income Eligibility Documentation
2023 Current U.S. Individual Income Tax Return or W-2 from both parent/guardians in the home
Last 4 weeks of paycheck stubs showing year-to-date gross totals for ALL jobs in the current year from both parents or guardians in the home
Section 3: Proof of East Haven Residency
Mortgage or Lease Document
TWO (2) Current Utility Bills in the name of the parent/guardian (dated last 30 days)
If you are currently living with someone else, or do not have a lease or mortgage in the parent/guardian's name, please call the main office.
Sección 1: Documentación del niño
CT Registro de evaluación de salud de la primera infancia y registro de vacunación
Vacuna contra la gripe (obligatoria en el otoño antes del 1 de enero de 2025)
Documentos de custodia o tutela, plan de crianza y/u órdenes de protección (si corresponde)
Documentación sobre discapacidad o necesidades especiales (Ejemplo: desde el nacimiento hasta los tres años, IEP, plan 504, servicios del habla o de OT) (si corresponde)
Sección 2: Documentación de elegibilidad de ingresos
Declaración de impuestos sobre la renta individual actual de EE. UU. de 2023 o W-2 de ambos padres/tutores en el hogar
Talones de cheques de pago de las últimas 4 semanas que muestren los totales brutos del año hasta la fecha para TODOS los trabajos en el año en curso de ambos padres o tutores en el hogar
Sección 3: Prueba de residencia en East Haven
Documento de hipoteca o arrendamiento
DOS (2) facturas de servicios públicos actuales a nombre del padre/tutor (con fecha de los últimos 30 días)
Si actualmente vive con otra persona o no tiene un contrato de arrendamiento o una hipoteca a nombre del padre/tutor, llame a la oficina principal.
#3 Child Screening / Evaluación infantil
Every new student need to have a preschool screening. Overbrook uses the Sparkler app.
You can complete the preschool screening at home, or make an appointment to complete it with a staff member at school.
