 Your child must be 2.8 years of age by

January 1, 2022

. Birth Certificate

. Yellow Early Childhood Health Assessment

Form with current immunizations and flu shot

. Parent or Guardian’s photo ID or Passport

. Proof of East Haven Residency

(2) current utility documents

(1) Mortgage or lease agreement

. Proof of Income

(4) current paystubs of parents

living in the household or 2021 W2 form
