If you would rather print the application and turn it in by hand, you can follow this link: Reassessment Application
Additionally, I have a pile of paper copies at school on the back table that you can pick up when you are in the classroom.
Almost every rule shown below is to support you in your learning and help me to see that you know what you are doing. When you complete a reassessment application you are doing the hard work of learning the material. I want you to come in for the quiz question knowing that you'll do better, not hoping that you'll do better.
You need to know exactly which standard you would like to reassess when you apply. I want you to be aware of what you are learning so you are able to make connections.
You need to show all steps when correcting quizzes or doing extra practice problems. When doing additional notes, remember: more is better. I want to make sure you know exactly what you are doing on every problem so that I know how well you understand the concept. The application is the hard work of learning the material and I need to see that you fully understand things.
All work that you turn in for the reassessment will need to be at a proficient level (that means all problems need to be at a 3-4 level). You need to show that you really get what's going on and are able to work these problems at a high level.
When you turn in the application, you will have a minimum of 2 school days until you are allowed to come in to do the reassessment (for example: if you turn in the application on a Thursday, then Monday will be the earliest that you can take the quiz). I need time to make sure I can check over your work properly and you don't just cram for the quiz and forget everything that afternoon.
You can only take one reassessment per day. I do not want anyone to sandbag the whole semester and come in on the last day to do 12 reassessments. A useful skill in your future is the ability to plan in advance and this is a place to practice that skill.
I will not walk through things with you on a topic that you are doing a reassessment on that day. I am more than happy to help you on the topic, but we will have to reschedule the reassessment to a different day. I really want you to know the material and I don't want you to just cram for the quiz and then forget everything.
You cannot take a reassessment during class time. We are covering new material during class and you need to be present (both mentally and physically) to learn it. I do not want you to get behind on current material while you are trying to understand older material.
Turn in reassessment applications either to the trays on the back table OR on the Form below. I will try to check the trays daily and the Form will send me an email when a person turns in an application. Do not put your application randomly on my desk or it may get lost in the shuffle of paper.
Once I have had time to look over your application, I will email you information (this will either be me asking you for a time/date to come in or will be further instructions and questions about mistakes you are still making).
You have up to 3 weeks from receiving my email about coming in to do the reassessment to complete it. If you do not complete it in that time, you will have to go through the process of completing a new application all over again. I do not want you to forget the material you were trying to get better at understanding and come in hoping you will do better, I want you to know the topic and come in knowing you'll do better.
Check the class calendar so you know when the last day you are able to complete a reassessment. This is always before finals week starts, so plan accordingly.
You will need to fully complete the Form below to turn in your work if you would like to submit your application digitally. If you prefer to use paper, you can pick up the application on the back table in my classroom OR print the application from the link at the top of this page.
You need to be signed in to your school Google account to see the application below. If you are not currently signed in, open a new tab and go to Google. Log in to your Durant school account. Then come back to this tab and reload it to see the application below.
