Submit to the ST Secretary on or before the deadline posted.

• EAF of each proponent (to be submitted only ONCE per term)

• 3 ring bound copies of the thesis document signed by the adviser (and the co-adviser, if any)

  • Methodology must now include the ff:

    • Data collection instruments to be used (survey form, interview questions, software/hardware instruments)

    • Participants' profile

    • When and where will data be collected

    • What data will be collected

    • If applicable, material used to acquire participants

    • If applicable, statement describing compensation for participation

  • Turn It In similarity report

  • Appendix A : Research ethics

    • Research ethics checklist

    • If applicable, informed consent form
