This is the official DST thesis template. Students must follow this template all throughout thesis stages THSST1, THSST2, and THSST3.
MOTA (Most Outstanding Thesis Award) Guideline
The student group should consult this document if it aspires to be nominated for MOTA, or if it has already been nominated. MOTA nominations are decided unanimously by the group’s adviser and panel members after a successful FINAL defense, during THSST3.
The Thesis Change Request Form is used by students to formally request any modifications to their thesis.
At the top of the form, you should indicate your current thesis title and the date of submission. Next, specify the type of change by checking the appropriate box (e.g., title, group members, objectives, scope, or others). Use the table provided to clearly show what is being changed, what it will be changed to, and the justification or reasons for the request. Supporting documents may be attached if needed. Once completed, the form must be noted and signed by your thesis adviser and panelists.
After their review, the thesis coordinator will evaluate the request, mark it as either approved or disapproved, and provide any remarks or comments. Finally, submit the accomplished form, with all required signatures and attachments, to the thesis coordinator (jazzie.jao@dlsu.edu.ph). Incomplete or unsigned forms will not be processed, so make sure all sections are properly filled out.
THSSTx Frequently Asked Questions (FAQs)

General Research Ethics Checklist
This checklist is to ensure that the research conducted by the faculty members and students of De La Salle University is carried out according to the guiding principles outlined in the Code of Research Ethics of the University.
All student groups are required to fill this up.
Research Ethics Review Form (ver Oct 2024)
The purpose of this form is to serve as a replacement for the current Ethics Clearance Form used during thesis defenses. It functions as a mechanism for panel members to formally indicate that they have reviewed the group’s research design and methodology, and have found no potential ethical risks that would necessitate a formal review by the Research Ethics Office (REO) or justify the rejection of the thesis topic.
Review of possible ethical issues is done before a research is allowed to commence, thus, this form should be accomplished for ThesisProposal / THS-ST1 defense.
All student groups are required to fill this up, make sure signatories are complete.

📗 THS-ST2 Files
This is a final deliverable requirement that the student group must submit. The form must explicitly contain the grade recommendation for the group. Make sure to double check the form is completely filled up.
📘 THS-ST3 Files
College and Panel Approval Sheet
Only Approval Sheets with complete signatures (even by proxies) will be accepted as a valid submission to the Office of the University Registrar. All authorization letters must be submitted simultaneously for signatures by proxy.
Departmental Record of Contribution
The contribution disclosed herein will formalize the contribution of each of the proponents and the adviser to the research. It is important that the proponents and the adviser agree with the figures and remarks imparted here, and it is expected that all has arrived at the same conclusion in good faith.
Make sure this form is completely signed.
DLSU Record of Invention and Assignment of IP
The purpose of this DLSU Record of Invention Form is to record the creation of University-owned intellectual property of university employees and students and the circumstances under which it was created or other intellectual property assigned to the University. It provides a written record of the invention to the DLSU Intellectual Property Office (DIPO) about patentability, possible drafting of a patent application, commercialization and/or for registering a mark or a work protected by copyright.
Please consult with your adviser if your group is required to submit this form.
The Document Sharing Policy Form establishes how a thesis research document may be shared after submission and defense. It ensures that the rights of the student proponents, adviser, and department are respected while also clarifying the conditions under which the document may be accessed. This policy applies only to the written research document. It does not apply to other submission requirements such as brochures, posters, or video presentations.
