Step 1: Read and understand the Parent Handbook linked below before registering.
Step 2: Register your child online using the following LINK.
Step 3: Complete the necessary forms for medication administration and/or extracurricular release form, if required, and return to a PE BASE staff member.
Medication Release Form: For children who receive medication during program hours. Must be signed by Primary Care Physician. Click HERE to access the form or request one from a P.E BASE staff member.
Extracurricular Release Form: For children who participate in extracurricular activities while attending the program. Ex: Band, orchestra, etc. Please request a form from a PE BASE staff member.
Please allow at least 48 hours for P.E BASE to approve enrollment.
