ADMISSION TO SCHOOL:

As students come to campus, there are 2 screenings that will be conducted:

1) Temperature check

2) Screening for the 3 questions to rule out Covid symptoms and potential exposure. For Elementary we will use a monthly form with DAILY check-ins.

Side 1 -- Parent answers 3 questions and signs/dates (1 time only for the month).

Side 2 - Parent signs EACH DAY prior to coming to school (Prior to boarding bus or getting out of car). By signing p. 2 EACH DAY a student is coming on campus, you are answering NO to the following 3 questions:

      • Have you been in close contact (within 6 feet for at least 15 minutes) with any person diagnosed with COVID-19 or quarantined by the health department or a health care provider?
      • Do you have any of the following symptoms: fever, chills, shortness of breath or difficulty breathing, new cough or new loss of taste or smell?
      • Have you been diagnosed with COVID-19?
    • Parent/Student ATTESTATION FORMS will be ORANGE and will be transported back/forth in the plastic back of the student take home folders. We will check for these daily as we take temperatures prior to allowing entrance into our school facilities.
