Exposure Occupation Information is as follows. Required exposure information is bold.

      • if providing principal information (if claimant was a bystander) use code 201

  • Occupation Other Name

    • • If no Occupation Code exists, provide the occupation at the time of exposure to USM asbestos-containing products.

  • Occupation Other Desc

    • • If no Occupation Code exists, provide the occupation at the time of exposure to USM asbestos-containing products. Describe the nature or circumstances of exposure to USM asbestos-containing product(s):
