  • Yes or No answer to: Did the injured party ever smoke cigarettes or cigars?

• Employer Begin Date

• Employer End Date

• Smoking Quit Date

• Smoking Product

  • Valid options for smoking product:

    • Cigarettes

    • Cigars

• Smoking Frequency

  • Number of pack per day (use decimal)
