Medicaid

Individuals of all ages with a disability may be eligible for New York State Medicaid benefits.  In some cases, Medicaid eligibility is an essential step in the transition planning process for students planning to exit high school into adult services.  Please contact Kelly Sullivan (631-730-4939) to set up an appointment to review the eligibility requirements and a review of the Medicaid Waiver programs and services currently available.
