When someone expresses the thought of killing themselves, it naturally raises red flags. Knowing what to do and when to do it is essential. If a family member, friend, or colleague has expressed suicidal thoughts or feelings - take it seriously.

I assess the danger of the situation by determining where the person is in his or her suicidal thinking. The danger grows with each step:

  • Ideation: expressing the thought of killing oneself or the desire to be dead

  • Intention: indicating a clear intent to hurt or kill oneself

  • Plan: talking about how he or she will do it

  • Means: having the tools necessary to carry out a plan (for example, purchasing a gun or accumulating pills)

    • Time line: setting a near-term date (for example, "I'll be dead by the weekend.") If someone has called you to say goodbye, CALL 911 RIGHT NOW!

    • suicide is often attempted "spur of the moment" and the person may not have a premeditated plan or time.
