NORTHWEST MIDDLE PE / HEALTH PARENT LETTER

I must have this completed form in order for your student to participate.

• Northwest Middle PE / Health requires the students to come to class dressed out every day during class to be able to participate safely. Dressing out consists of the following: tennis shoes, and socks.

• Students need to be willing to participate in ALL activities and show good sportsmanship during class.

• Students need to follow directions the first time they are given. Failure to do so will result in a disciplinary action (detention, parent contact, or referral).

• Students must bring a note from a parent if they are too sick to participate, but regardless they MUST DRESS OUT.

• If an assignment is missed, five days will be allowed for make up. 

• Bring a note from the DOCTOR if you have an injury or illness that will last more than 2 CLASS PERIODS.

• Grading will consist of the following:

  • %50 Minor = dressing out, participation, quizzes, etc

  • %50 Major = skill and knowledge tests

• Every student in SC is required to take health during PE each year in middle school.

• For more information during the year, the teacher’s website may be used as a reference.

Classroom expectations: 

1) Students must dress out: your grade depends on it. 

2) Student involvement and engagement is a grade as well. :) 

3) Follow directions: the behavior steps depend on it. :) 

4) Be respectful to the teacher: the behavior steps depend on it. 

5) Do not talk while I am talking or peers are talking. 

6) Respect equipment.

7) No phones: referral. (School wide rule). 

8) No food. No squirt bottles. 

9) Be respectful to each other.
