Medication Policy
Zumbro Education District acknowledges that some students may require prescribed or over-the-counter (OTC) medications during the school day. Our licensed school nurse, health office aide, or other trained staff will administer any prescribed or OTC medications in accordance with law and district policy and procedure.
Medication Authorization forms must be filled out and signed for each medication to be administered at school.
Medical Provider signature MUST be provided for any prescription medication
OTC medications can only be given at recommended dosages unless otherwise noted by medical provider.
A Release of Information form must be signed for communication between the school health office and medical provider.
Medication MUST be brought to school by parent or other designated adult. Medication cannot be sent on district transportation.
Medication MUST be brought in original packaging and stored in the Health Office.
Students are not allowed to self-carry medication unless signed off by Licensed School Nurse AND parent/guardian.
Prescription medication must be in a pharmacy bottle with correct prescription information (extras for school can be requested at pharmacy)
OTC medications must be brought in in original and UNOPENED containers
Any prescription or OTC medication unclaimed or abandoned at the end of the school year will be properly discarded in a designated drop-off box. In the case of a controlled medication, a request to law enforcement will be made to transport medication to the designated drop-off site.
