Minnesota State Law requires parents/guardians to provide proof of immunization, or appropriate documentation exempting the student from such immunization, and such other data necessary to ensure that the student is free from any communicable diseases, as a condition of enrollment. While we encourage that all of our students are up to date on vaccinations, we understand and respect that there are some that are unable or choose not to vaccinate. If you choose not to vaccinate your student, please print this IMMUNIZATION FORM and have the second page signed and notarized, listing vaccinations not received.

7th GRADERS are required to have a Tdap and Meningococcal vaccinations before beginning the school year.
