Immunizations

According to the World Health Organization, "Immunization currently prevents 3.5-5 million deaths every year from diseases like diphtheria, tetanus, pertussis, influenza and measles." Not only do immunizations protect those we love and care about, they also protect future generations.

Minnesota State Law requires parents/guardians to provide proof of immunization, or appropriate documentation exempting the student from such immunization, and such other data necessary to ensure that the student is free from any communicable diseases, as a condition of enrollment. Please see the charts below to see at what ages immunizations are required. 

Vaccination Exemptions:

While we encourage that all of our students are up to date on vaccinations, we understand and respect that there are some that are unable or choose not to vaccinate. If you choose not to vaccinate your student, please print this IMMUNIZATION FORM and have the second page signed and notarized, listing vaccinations not received. 


Finding Vaccination Locations:

If you need help finding places that offer immunizations and don't have a current medical provider, please refer to the "WHERE TO GET IMMUNIZATIONS" sheet.
