This is the new form that your health care provider will need to fill out if your child will be taking any medication at school this year.

This form is NEW so even if you've had it approved in the past, you'll need to get new authorization from your child's doctor before we can accept their medication in the clinic. Please print this out and have your doctor fill it out. You can fax it to us at 941-255-7477, or simply bring it in when you drop off their medication. Our school nurse will be at our Beep and Greet event next Wed. the 26th from noon to 6pm if you want drop it off then.

One new policy for this year...we are unable to administer nebulizer treatments in the clinic. We can still have inhalers on hand, but we are unable to give a child a nebulizer.
