Where there is a change to your project following approval, you should submit one of the following forms, depending on the signifiance of the change.
Common examples of these are:
change of project end date, or study timeline.
additional ‘research sites’ (i.e. places where research activities are taking place) but which are fundamentally the same as those set out in the original application (or previous amendment).
minor changes to data collection methods or tools (e.g. using on-line as well as paper questionnaire).
changes to the research team.
To request approval for these changes, please submit the following:
the minor amendment form.
a copy of the ethics application form which the request relates to.
a copy of the ethics approval email / form from the application.
Typical examples of changes requiring ethical approval are:
changes to the study design (e.g. additional/removal components, changes to/additional research participant groups)
changes to data collection methods
or any other aspect of the research that may increase risk of harm or distress to either participants or researchers
To request approval for these changes, please submit:
a copy of the ethics application form which the request relates to.
a copy of the ethics approval email / form from the application.
Any other study documents relevant to this application.
