Withdrawal

STEPS TO WITHDRAW FROM A SHSM PROGRAM

1. Complete and fill out the Google Form for withdrawal below.

Once the form is completed, please do the following: 

1. Print it out (a copy will be emailed to you after you fill it out)

2. Student and parent/guardian should sign the withdrawal form 

3. Find your SHSM Liaison Teacher to sign the withdrawal form

4. Submit the application form to the Guidance Office Secretary (Ms. Marino)
