STEPS TO APPLY TO A SHSM PROGRAM

1. Follow the steps outlined in this video to declare interest in a SHSM program on My Blueprint: Declaring Interest on My Blueprint 

2. Complete and fill out the Google Form Application below.

Once the form is completed, please do the following: 

1. Print it out (a copy will be emailed to you after you fill it out)

2. Student and parent/guardian should sign the application form

3. Submit the application form to the Guidance Office Secretary (Ms. Marino)
