Screenings
Screenings will be performed throughout the course of the year as per New Jersey state requirements. If your child has an annual physical performed by an MD/DO, NP, or PA which includes screenings, your child may not need to be screened in the health office. Please submit a copy of their physical to the health office. I will notify you of any screening findings which fall outside expected parameters for follow up with your medical provider.
The New Jersey regulations for screenings, as per N.J.A.C. 6A:16- 2.2(k), are:
Screening for height, weight, and blood pressure shall be conducted annually for each student in preschool through grade 12.
Screening for visual acuity shall be conducted annually for each student in preschool and biennially for students in kindergarten through grade 10.
Screening for auditory acuity shall be conducted annually for students in preschool through grade three and in grade seven and 11 pursuant to N.J.S.A. 18A: 40-4 .
Screening for scoliosis shall be conducted biennially for students between the ages of 10 and 18 pursuant to N.J.S.A. 18A:40-4.3.
Screenings shall be conducted by a school physician, school nurse, physical education instructor or other school personnel properly trained.
The school district shall provide for the notification of the parent of any student suspected of deviation from the recommended standard.
Immunizations
The following are New Jersey state mandated vaccinations for students entering a New Jersey public school in grades K-5:
4 doses DTP (Diphtheria, Tetanus, Pertussis) with 1 dose given after 4th birthday or 5 doses
3 doses Polio (OPV) with 1 dose given after the 4th birthday or any 4 doses
2 doses of MMR (Measles, Mumps, Rubella)
1 dose of Varivax/Varicella (Chicken Pox) dose must be administered on or after 1st birthday
3 doses of Hepatitis B (series of 3 injections)
