Filing, Submission & Request

In the Google Form, fill in necessary details for the report.
Review your responses before submitting. Everything must be filled out to submit the form.
After submission, an email with a PDF attachment will be sent to you stating that you have successfully filed a report. The same PDF will be sent to the Discipline Office.
*For AR submission, you will not be able to see the attachments you uploaded in the Google Form. Only the Discipline Office can access them.
Go to the Discipline Office. Copies of the three forms are available inside. You can also download and print the form by clicking the image of the desired form.
Fill in required information. Don't forget to affix your signature.
Review and submit your form in the appropriate shelf.
Click the "Request DOcuments" button below the Lost and Found Item Claim Form.
Fill in the form then submit.

An anecdotal report is a narrative account of the incident, detailing the date, time, place, persons involved, and interventions or actions done, if any, by the one filing the report.
Any member of the school community who witnesses, discovers, or learns of the occurrence of a possible violation of the PSHS Scholar's Code of Conduct may file the report. The person filing the report must affix his/her signature on the report.
The anecdotal report should be submitted to the Discipline Office within three (3) school days from the commission or discovery of the incident.
If an intervention is done, as in the case of Level I offenses, details of the intervention(s) should be indicated in the anecdotal report. The report is signed by the school authority who implemented the intervention(s) and is filed with the Discipline Office.
