Parent Bus Release Form:
The Student athlete is responsible for completing the form below with the required administrator signatures and submitting to the Weber High office 24 hours in advance. All late submissions will be denied and the player will be required to ride the bus to and from the games.

Sam Pluim is doing our home game player pictures again this year. Below is a sign up sheet:
4 Players per game
$55 per player, per game
15+ pictures per player, per game
Please contact Sam Pluim801-648-0315 email pluim.media1@gmail.com if you would like to look into this option
Sam usually gets pictures of more than just the 4 players and will send us a link to buy whatever he shoots.
