New Student Registration

NEW STUDENT REGISTRATION CHECKLIST

once ALL required documents have been received, we will continue with enrollment and scheduling.

• Student's Official State Birth Certificate with notary/seal

• Student's Current Immunization Records || Immunization Requirements: ENG || SPA

• Parent/Guardian's current Photo ID  (Only parent/legal guardian can register a student)

Driver's license, passport, etc.

• Proof of Residency (2 forms) in parent/guardian’s name: 

Utility Bill (dated within the last 60 days), Closing Documents on a new mortgage, Lease/Rent Agreement, or Residence Disclosure Form 

• Completed RJH Registration forms (pick up in Counseling Office or download below; completed forms can be dropped off or emailed to stmoore@wsd.net)

7th Grade:  School Year 2023-2024   School Year 2024-2025

8th Grade:  School Year 2023-2024   School Year 2024-2025

9th Grade:  School Year 2023-2024   School Year 2024-2025

If applicable:

• Medication Authorization form (MUST be submitted if your student requires prescribed med(s) during school hours)

• Homeless Student Identification form

• Residence Disclosure form

• Free or Reduced Price Lunch Application form (Faster Online Method)

• 504 Documents

• IEP Documents

• ELL Documents

• Guardianship/custody court documents

FOR STUDENTS TRANSFERRING MID-YEAR

You will need all items listed above, and:

• Official Withdrawal Form from your student's previous school
