Administration of medication will be permitted at school only when medically necessary and under the direct supervision of the school nurse. Medication should be given outside of school hours whenever possible. Before any prescription medication may be given at school, these policies must be followed (Policy JLCD)
Any medication that is to be administered at school MUST be brought in by a parent/guardian/adult. Students are not allowed to bring medications in on their own, this includes any over the counter medications. Students are not allowed to carry or self-administer medications.
1. Request For Prescription Medication Permission Form.
2. Parent or Legal Guardian portion of the form must be filled out and signed.
3. Physician portion must be completed and signed for all prescription medications, or a physician's order.
4. All medications must be brought to the school by the parent or other adult in the original container and appropriately labeled.
Over the Counter Medications such as Tylenol and Ibuprofen, can only be administered with parent/guardian written consent. OVER THE COUNTER MEDICATION PERMISSION FORMS NEED TO BE FILLED OUT YEARLY. You can find the form HERE
If your child was on a medication during the school year, whether prescribed or over the counter, you MUST PICK UP the unused medication in the health office by the last day of school. If you do not pick up unused medication it will be discarded. NO medications will be sent home with students
