Step 1: Complete Application and Physician Form
Step 2: Have the employer sections completed and signed by the place you plan on working.
Step 3: Return it to WR to High School Office
Step 4: HS Office will file your application and print the ACTUAL permit from the Ohio Department of Commerce website.
