In Vermont, exemption from vaccination requirements are allowed for religions and medical reasons.

• The Religious Immunization Exemption Form MUST be signed by the parent ANNUALLY and returned to the Health Office .

• The Medical Immunization Exemption Form MUST be filled out by the health care provider and returned to the Health Office
