Please contact the health office with questions regarding the health forms.

Forms need to be completed yearly.

Feel free to use the health form here or call me at (802) 874-5224, email me at desirae.wilbur@wnesu.com, or send me a Remind message, to request a form to be mailed or sent home with your student. Prescription medication forms can be faxed to the school by your child's health care provider. Health forms are available to the left.

If your student has a current asthma diagnosis or uses an inhaler regularly, an Asthma Action Plan should be submitted by your physician. If your student is going to be taking Prescription/ Non-prescription Medication at school, a Prescription Medication Permission form must be completed by you and your student's primary care provider.

For students with life threatening allergies, an Emergency Action Plan should be on file for your student and renewed annually. Your physician's office can do this at your request, often without an additional exam. Plans can be faxed to the school 802-843-2911.
