Student School Account Required to Access Form

Please ensure it is completed no later than

Friday 25th April 2025 at 4:00pm

All fields need to be completed and reserved choices put in for each option choice.

If you need any further support with this, please contact the relevant college.

Important:

Please ensure that your child completes the form in full, with a parent/carer present to support with the process.
The Google form can only be accessed through a student Google account. If you have any difficulties with this, please contact the relevant college admin team.
We recommend that the form is only completed once you have read through the subject guides, met with staff at our parents' evening and reviewed your report.

Note: Students can only complete and submit the form once.

The Curriculum Choices form will be sent to all students on Wednesday 26th March 2025.
