Medical Information

Travel Doctor Visit:

Heading to the travel doctor or doctor of infectious diseases before heading on a trip abroad is recommended but certainly not required.

To find out about any concerns there might be in the Dominican Republic at this time, please visit the Center of Disease Control website at: cdc.gov

Once on the CDC website, click on the Traveler’s Health tab and Destinations. Type in Dominican Republic for the destination and simply mark “Visiting Family and Friends” for type of traveler. Suggested vaccinations should come up. It is your choice whether or not you take your child to receive these vaccinations or other medications.


Required Medical Forms:

In order to best serve the needs of the students I am traveling with I must have a copy on file of any and all medical conditions, allergies and current prescriptions and over the counter medications that they will have with them on the trip. In the event of an emergency where medical care is needed I will provide translation to the medical staff of any an all medical concerns.

This form has been given to your child and must be turned in to me before June 1st, 2019.
