Medication

Prescription medication requires a written order from the physician/healthcare provider or pharmacist stating the name of the medicine, the dosage and the time it is to be given.  Over the counter medication (OTC) does not require a physician's order.  Written permission from the parent or guardian is required for all medications to be administered at school.  

WESD does not stock over the counter medications for students.  We do not have Tylenol, Advil, Neosporin, etc. on hand.  All medications that need to be administered to students, must be provided by the parent or guardian.  Cough drops are considered an OTC medication.

A signed physician's statement indicating the necessity must accompany any request for self-administration of medication, whether it is prescription or over the counter, except in the case of medication for diagnosed anaphylaxis and breathing disorders requiring handheld inhaler devices.  The self-carried inhaler or epi-pen must be labeled by the pharmacist with the student's name, the physician's name, name of medication, dose and frequency of use.  Student misuse of self-administered medication may result in disciplinary action, and seizure of medication.   

See attachments to print the request giving medication at school and/or the self-carry consent form.  Bring the completed form(s) to the health office with the corresponding medication in the original pharmacy container.  Please note that the consent for self-carry medications needs to be signed by the student and pass a competency demonstration.
