When you visit the doctor or are hospitalized for any reason, we will also need a note simply verifying that you are able to fully participate in your chosen sport. The note may also list limitations if needed, but should include the exact date that those will expire.

ORTHONC'S SPORTS PHYSICAL NIGHTS!
In our commitment to the athletes and sports programs in the surrounding area, OrthoNC is hosting multiple sports physical nights at both our Wakefield and Knightdale Locations throughout the summer.
As always, our sports physicals cost $10 (exact cash, or check - no credit cards will be accepted)
All proceeds will be donated back to your schools if you have 10 or more athletes participate during these dates throughout the summer!
