New for 23/24 - the medical history form will now expire at 395 days (not per school year), When you redo your forms for the year, or anytime you upload a new physical please just update this form as well.  It will not automatically make you do this until the form is officially expired.

Dragonfly will tell you that your account needs approval.  You may fill out your forms while you are waiting for this approval (please do so!).  Please be sure to fill out each section (we do not fill out any sections so please DO NOT choose the option that states "my school will complete this requirement"), once they are all completed you will be approved for workouts.

To attend workouts you will need to complete all your online forms filled out with DragonFly Max, a new medical history for this school year, and a current physical on the updated physical form including the medical eligibility form.  All physicals after 6/1/21 will be required to be on the new form.    All the forms are linked on this page and in Dragonfly.  Physicals are valid for 365 days.
