Once your child is identified as gifted, you'll receive a lot of paperwork. Please save these documents for your records, as they are proof of your child's identification, especially if you move to a different school district in the future.

This form will give your consent to allow your child's data to be reviewed by the School Based Committee for Gifted Education to determine if Gifted Services are warranted.
This two-page form provides the results of the identification meeting. This letter will state whether the child has been placed in the gifted program or the talent development program, and it will outline the specific gifted label the child received according to the data used for identification. Once signed, this form gives the school permission to serve your child.
Individualised Service Plan
This document outlines the data that was used to support the need for gifted services, if the School Based Committee for Gifted Education was able to determine that the criteria for giftedness was met during the referral process. If so, the nomination resulted in identification with a formal AIG, IG, or AG label. If not, the student would have been placed in the Talent Delvelopment program.
Data in 3D Rubric or EOG Rubric
Occasionally, a student may require a more extensive data set in order to identify. In those instances, a rubric outlining a more comprehensive points evaluation system is used. For those children, a rubric form is also included with identification paperwork.
Differentiated Educational Plan
This document is created annually by the AIG teacher and the Classroom Teacher to outline the types of differentiation that will be provided to the individual gifted student during the course of the school year. A different document is issued each year and it can change with the developing needs of the child.
