Student Chromebook Device & Use Fee Acceptance/Waiver Form
The Wayne Township Public School District is excited that your child will be participating in our 1:1 mobile technology learning program for the 2019-2020 school year. This year, students in grades 9 & 12 will be issued a Chromebook device for their use in school and at home. Please review the below provisions of this program as well as the attached applicable Board of Education Policies and accept this form using your Capture Point account.
When using the district-provided Mobile Technology Device, students must comply with the District’s Acceptable Use of Computer Network/Computers and Resources policy and regulation: Board of Education Policy/Regulation#2361 (attached).
The district-provided Mobile Technology Device is, at all times, property of the District. The device and peripherals, loaned to a student, must be returned to the District in the condition they were initially provided to the student considering reasonable use and care by the student while they are a student in a District school. The District reserves the right to demand return of the device/peripherals at any time. Failure to return the device/peripherals may result in disciplinary action and/or criminal charges sought against the student and/or the person in possession of the device/peripherals.
The parent or student shall be responsible to reimburse the District the cost repair or replacement of any technology device/peripherals that are lost, damaged beyond reasonable use or beyond their value, abandoned, missing, stolen, or cannot be returned to the District. (Also see Technology Use Fee Option below.) “Damaged” is defined as hardware or software breakage that hinders the operation of the device while being used within the parameters of the Acceptable Use policy.

The electronic device provided by the Wayne Township Public School District may record or collect information on a student’s activity or a student’s use of the device. The district may remotely activate a location tracking feature. The district may monitor device usage and internet traffic anytime the device is connected to an internet connection. The Wayne Township Public School District will not use any of these electronic capabilities in a manner that would violate the privacy rights of the student or any individual residing with the student.
The district uses a software to help monitor student’s Google Apps activity. Gaggle.Net is a safe, web-based internet system for students. All messages pass through a number of filters. Messages with offensive language, questionable domains, banned files types, or pornographic images are automatically rerouted to the student’s Assistant Principal for review. Students will be warned directly via email for other less serious infractions.
Technology Use Fee
As with any district-owned property, the district-provided technology device should be handled with care regardless of where it is being used. In accordance with Wayne Township Public School Board of Education Policy #7523 (School District Provided Technology Devices to students), the District is offering to students/parents a technology insurance use fee to protect student/parent from bearing the full cost of repair of accidental damage of the device and/or its peripherals.
Payment of a, one-time non-refundable $30 use fee covers ONE accidental damage incident to the district issued device. After initial damage and repair, the parent/student will be offered an additional $30 insurance technology use fee to cover the cost of another repair for accidental damage. Any parent /student who waives to not purchase the technology insurance use fee will be responsible for the cost of repair to the device (See price list below). All repairs to the devices will be done by the district.
All damage to a device/peripherals must be reported to the Assistant Principal’s Office immediately and a technology ticket entered by the student through the Library Media center Technology Kiosk Station.
Lost or stolen devices/peripherals are not covered by payment of the use fee. In these cases, students/parents are required to pay the full cost of the replacement of the device/peripherals (See price list below).
Damage caused intentionally or by inappropriate usage that violates the District’s Acceptable Use of Computer Network/Computers and Resources policy (#2361) or the School District Provided Technology Devices to students policy (#7523) is NOT covered by payment of the use fee.
