Date: Friday May 30th, 2025
Time: Appointments will begin at 4:00 PM and go through 6 PM
Location: Watauga High School
Free Sports Physical for all rising 6th - 9th grade students

How to reserve an appointment:
1. Print HOP packet by clicking the link above (visit your school if you need help printing)
2. Complete HOP packet
3. Return the packet to your school for appointment time
