The Home Hospital Instruction program coordinates academic instructional service for students in the Walnut Creek School District who have a temporary disability, which makes school attendance impossible or inadvisable. These students shall receive individual instruction in their home, in the hospital, or other residential health facility, excluding state hospitals. This instruction applies to students incurring a physical, mental or emotional disability after which they can reasonably be expected to return to regular day classes, or an alternative education program without special intervention. The program is designed to keep students current with their regular instructional program. Credentialed teachers provide instruction on a one-to-one basis. A student who is enrolled in the program shall receive up to five hours of instruction a week based on the student’s health care provider’s recommendations. Students are provided instruction for all required subjects based on availability of teaches. Teachers, office personnel, and administrators need to be aware of the possible need for individualized home instruction whenever a student has a temporary disability. Home Hospital Instruction is initiated by a request from the parents. Many parents, however, may not be aware that this option is available to them. Consequently, whenever a student has been out with an illness or injury for more than one week and there is no indication that the student will be able to return to the classroom soon, the option of Home Hospital Instruction should be brought to the attention of the parents. A doctor must fill out the Request for Home Hospital instruction form for the process to begin.
GUIDELINES FOR HOME HOSPITAL TEACHERS
To Begin Home/ Hospital Instruction:
The Student Support Services department will contact you and send you a packet containing:
· Daily Attendance Record(s)
· School calendar
· Time sheet(s)
· The Home Hospital Program Guidelines
You will be sent an email introduction for you, the parent, the school administrator, the counselor (if in middle school), the attendance clerk, and the teachers. Once introduced, you are responsible for contacting the student’s school and each teacher for work to be covered, books and materials. You are entitled to one hour of payroll time for weekly procurement of books/materials. The Director of Special Eduation approves all timesheets. Materials may be picked up at the school. When books/materials are issued for the Home Hospital Instruction student, the school may require you to sign out for them. Make contact with the student’s parent/guardians to establish a schedule for Home Hospital Instruction that is agreeable to all parties.
Determination of Hours of Instruction: Each student is entitled to five (5) hours of Home Hospital instruction per week. You can only be paid for a maximum of five (5) hours of instruction per week per student, plus the weekly procurement of one (1) hour. Maximum hour of instruction per day should never exceed three (3) hours. Home Hospital instruction should not be conducted on school holidays.
Guidelines for Filling out Reports: The Director of Special Eduation must sign the timesheet at the end of each payroll period. The payroll period runs from the 11th of one month to the 10th of the next. Timesheets are due on the 10th of each month and should be recorded in fraction form to the nearest fifteen minutes as .25, .50, and .75. You must complete the time sheet thoroughly, which includes totaling your hours for each day. The Daily Attendance Record is due on the 10th of each month. Incomplete paperwork will be returned for correction.
