Pre K Requirements

Required Documentation for Pre-K

Children must be four years on September 1 of the upcoming school year and must be a Georgia resident. The following documents for proof of age includes

Birth certificates

Passport

Hospital record of live birth

Acceptable Proof of residence includes

Current lease

Property tax notice

Homeowner’s insurance bill

Utility bill, mortgage statement

Other information required

All children enrolled in Pre-K must  have a Certificate of Vision, Hearing, Dental, and Nutrition Screening (DHR Form 3300) on file within 90 calendar days of entry.

Immunization Record (DHR 3231) must be on file within 30 calendar days of entry
