4-H Camp  Packet for Registered Campers

4-H Online System 

4-H Online houses your child(s) important information, such as health history and Emergency Contacts. You will also sign their Code of Conduct, pickup permissions, and Media Release to give permission for pictures to be taken during our camp week. Your camper cannot attend camp if they are not enrolled in 4-H Online.

**If you are in a 4-H Club or have completed your enrollment before today there is no need to to do this step. If you are unsure call the office or email Heather at hajohnson@vt.edu

Emergency Contact: Please list someone other than a parent. The parents will always be called first but if we are unable to reach you we have someone else to call to try and reach you.

Returning Camper Instructions 

BAND App is a mobile app that allows us to send out important information to all parents during and leading up to camp without having to individually contact each family. 

**When entering your name please put your name and then your campers first and last name in parenthesis. example: Heather Johnson(Ayden Johnson) **

Please use this form to designate who may pick your camper up from Jamestown on Friday, June 12th. 

Valid Picture ID will be required at time of pickup.

2026 Camp Pick Up Permissions

If your camper(s) have a cabin mate they would like to request please complete this form. Both campers need to request each other by May 15. We will try to honor late requests, but cannot guarantee. Notifications of cabinmates will not be made prior to camp. ***We will not honor requests if 4-H Online Enrollment is not complete***

2026 Cabin Mate Request Form

If your camper will be bringing prescribed medication or over-the-counter medication with them to camp, please complete a Medication Form (if you have not already uploaded to 4-H Online) and email to hajohnson@vt.edu. This includes inhalers and epi-pens. Remember that all medications brought to camp must be in original containers

Please print a copy of the medication form to bring with your medication at camp drop off.
