CCO Choice List-The Care Coordination Organizations identified here were formed by existing providers of developmental disability services to provide Health Home Care Management and Basic HCBS  Plan Support.

New York State Office for People with Developmental Disabilities OPWDD- The Office for People with Developmental Disabilities assists people with developmental disabilities and their families in gaining access to high-quality services and supports tailored to each person’s unique needs. 

Front Door-The Front Door is the way OPWDD connects people to the services they need and want. Once you enter, a person-centered planning process.

Medicaid Waiver-Programs that help provide services to people with developmental disabilities.  The NYS Office For People With Developmental Disabilities (OPWDD) handles Medicaid waivers for developmental disabilities.

Supplemental Security Income(SSI)-Supplemental Security Income (SSI) is a Federal income supplement program funded by general tax revenues (not Social Security taxes): It is designed to help aged, blind, and disabled people, who have little or no income; and It provides cash to meet basic needs for food, clothing, and shelter.

Travel Training-The following agencies provide transportation services including public, private and travel training. 

Guardianship-The Westchester Arc guardianship program helps families apply for guardianship and is available to answer questions from anyone residing in Westchester County concerning guardianship.

Westchester Independent Living Center-The Westchester Independent Living Center, Inc. (WILC) is a not-for-profit, community-based advocacy and resource center that serves people with all types of disabilities.

The ARC of Putnam-Is one of 46 chapters of The Arc of New York (formally NYSARC, Inc.), and provides advocacy and programming for children and adults with intellectual and developmental disabilities within the geographic region of Putnam County along with Northern Westchester and Southern Dutchess counties.
