7th graders are required to have a T-dap and Meningitis vaccine.
12th graders are required to have a second meningitis vaccine.
Kindergarteners need multiple updated immunizations prior to entry. Please refer to the Ohio Immunization Summary for School Attendance.
If your student is exempt from immunizations according to the Ohio Revised Code 3313.671, an exemption form will need filled out and returned to the school every school year.
