If your student requires medications to be administered at school, this form must be completed by the doctor. The form must include the medication, dose, route, and frequency. It must be signed by a doctor and a legal guardian in order for the school staff to be able to administer the medication. A new form will be required each school year. Asthma medications and Epi-Pens require a specific form. See below for those forms.
The medication must be brought in by the legal guardian in the original labeled packaging from the pharmacy. Medications are not to be transported by students. Exception: If the order states the emergency medication may be carried by the student (such as inhalers or epi-pens).
Anaphylaxis order and treatment plan: If your student does not go to Akron Children's, please print this form to take to your doctor if your child requires and EpiPen at school. Akron Children's Emergency Action Plans will be accepted. If your student needs an EpiPen at school, the school will need the medication provided by the parent unless the order states that they are a self-carry.
