Yardville Heights Elementary PTA COVID Guidelines/Waiver & HTSD Code of Conduct: Please Read and Sign Below:
Per the request of the H.T.S.D. B.O.E., Yardville Heights Elementary PTA is required to have a Covid policy in place for our events. Please review the YHS PTA Covid policy. The YHS PTA will follow Covid-19 guidelines set forth by the CDC and the district for all PTA related events. This will include but not limited to:
• All attendees will be required to wear masks inside any building that is being used for a PTA function regardless of vaccine status.
• Masks can be removed when eating, and then put back on after eating.
• Sanitizing stations will be provided throughout the event.
• Vaccinated or unvaccinated families who travel out of the tri-state area must follow Governor Murphy's Covid-19 Travel Guidelines.

• Waivers will be required to be signed before attending any PTA related event.
•. Families who have come into direct and/or close contact* of a positive case of COVID should not attend any event on school grounds.
*CDC definition of close contact:
•You were within 6 feet of someone who has COVID-19 for at least 15 minutes
•You provided care at home to someone who is sick with COVID-19
•You had direct physical contact with the person (hugged or kissed them)
•You shared eating or drinking utensils
•They sneezed, coughed, or somehow got respiratory droplets on you
Yardville Heights Elementary PTA COVID RELEASE OF LIABILITY, WAIVER, and ASSUMPTION OF THE RISK
I specifically assume all risks and hazards associated with my child’s participation in the PTA activity including, but not limited to, the risks associated with the COVID-19 or similar type virus. I understand that we will be associating with staff, volunteers, and other attendees through our participation in the activity. I understand and voluntarily assume the risk that myself and others participating may acquire COVID-19, and that COVID-19 may subsequently be transmitted from myself to my family, and members of my household. I certify that I am in good health, have no fever, and have no current issues that make it unsafe to participate in the event, which may not have a medical professional on staff. I will notify the school and not attend in the event that I develop a fever or illness or tests positive for COVID-19. To the fullest extent permitted by law, I hereby agree to waive, release, and discharge any and all claims, causes of action, damages, and rights of any kind, including serious injury or death, against the Yardville Heights Elementary PTA, its insurers, the school district, and all of their respective representatives, and volunteers (the “Released Parties”) arising from or relating in any way to my participation in the event. Including but not limited to exposure to the COVID-19 virus as we are assuming that risk.
H.T.S.D. CODE OF CONDUCT 5600:
In Accordance with the new NJPTA required Standing Rules, the Yardville Heights Elementary PTA will adhere to the H.T.S.D. 5600 Code of Conduct as Follows: Student discipline and the Code of Student Conduct will be applied without regard to race; color; religion; ancestry; national origin; nationality; sex; gender; sexual orientation; gender identity or expression; martial, domestic-partnership, or civil union; mental, physical or sensory disability; or by any other distinguishing characteristic, pursuant to N.J.S.A. 10:5.-1 et seq.o The Building Principal or designee shall have the authority to assign discipline to students. School authorities also have the right to impose a consequence on a student for conduct away from school grounds that is consistent with the district’s Code of Student Conduct pursuant to N.J.A.C. 6A:16-7.5. This authority shall be exercised only when it is reasonably necessary for the student’s physical or emotional safety, security, and well-being or for reasons relating to the safety, security, and well-being of other students, staff, or school grounds, pursuant to N.J.S.A. 18A:25-2 and 18A:37-2. This authority shall be exercised only when the conduct that is the subject of the proposed consequence materially and substantially interferes with the requirements of appropriate discipline in the operation of the school. Consequences pursuant to N.J.A.C. 6A:16-7.5 shall be handled in accordance with Policy and Regulation 5600, pursuant to N.J.A.C. 6A:16-7.1, and as appropriate, in accordance with N.J.A.C. 6A:16-7-2, 6A:16-7.3, or 6A:16-7.4. School authorities shall respond to harassment, intimidation, or bullying that occurs off school grounds, pursuant to N.J.S.A. 18A:37-14 and 15.3 and N.J.A.C. 6A:16-1.3, 7.1, and 7.7. o Note: The Board of Education may deny participation in extra-curricular activities, school functions, sports, graduation exercises, or other privileges as disciplinary sanctions when designed to maintain the order and integrity of the school environment, in accordance with N.J.A.C. 6A:16-7.1(d).N.J.S.A. 18A:6-1; 18A:36-25.1; 18A:25-2; 18A:36-19a; 18A:37-1 et seq.; 18A:37-13.1 et seq.
