Release of Liability, Waiver of Claims, Assumption of Risk, and Indemnity

Agreement

PLEASE READ CAREFULLY BEFORE SIGNING. THIS IS A LEGAL DOCUMENT.

1. Assumption of Risk

I understand and acknowledge that these Activities involve inherent risks, including but not limited to:

Slips, trips, and falls on uneven terrain or obstacles. Impact from paintballs, splatterballs, or laser tag

equipment. Equipment malfunction or misuse. Physical exertion which may cause injury. I freely and

voluntarily assume full responsibility for all such risks, known and unknown, even if arising from the

negligence of the Facility, its owners, employees, or agents.

2. Release of Liability

I hereby release, waive, discharge, and hold harmless Xtreme Paintball & Laser Tag, its owners,

employees, volunteers, and agents from any and all liability, claims, demands, actions, or causes of

action arising out of or related to any loss, damage, or injury, including death, that I may sustain while

participating in the Activities, whether caused by negligence or otherwise.

3. Indemnification

I agree to indemnify and hold harmless the Facility from any loss, liability, damage, or costs, including

court costs and attorney’s fees, that may occur due to my participation in the Activities.

4. Medical Authorization

In the event of injury, accident, or illness, I consent to emergency medical treatment deemed necessary

and agree to be financially responsible for any related medical costs.

5. Safety Rules

I agree to follow all posted rules, verbal instructions, and safety guidelines provided by staff. I

understand failure to comply may result in removal from the Facility without refund.

6. Minors

If the participant is under 18, this agreement must be signed by a parent or legal guardian. By signing,

the parent/guardian agrees to all terms on behalf of the minor.
