New patient form:
- Please print & fill out, if possible
Health History (+insur discl) 1.13.2016.pdf- Try to arrive 10 minutes prior to your appointment
- Bring your insurance card, glasses and contact lens boxes/peel packs for the doctor to see
- Bring your list of medications
- Bring sunglasses in case the doctor dilates your pupils (lasts 2-3 hours typically). Most patients are able to drive after dilation. If not, please arrange transportation.
- Please refrain from talking & texting on your mobile phone in our office
