Some documents require the signer to make an oath or affirmation. The notary administers an oath or affirmation by asking the signer to affirm or swear under the penalty of perjury certain truths (i.e. that they are in fact the individual signing, that they have read and understand the document, that they are signing the document of their own free will, that the contents of the document are true to the best of their knowledge, etc.).

Acknowledgements
With an acknowledgement, the signer has already signed the document and is acknowledging to the notary that the signature is theirs. An acknowledgement does not affirm or swear that the contents of the document are true and correct, only that the signature on the document is theirs' and they signed of their own free will.

Notaries may certify copies of vital records, including birth, death, marriage and divorce certificates, from other states.
The Sate of Colorado will not allow a notary to certify a copy of a Colorado birth, death, or marriage certificate. These copies must be obtained by the Colorado Department of Health state. Federal and/or other state documents can be copied as long as they do not state on their face that it is illegal to copy the record.

Often it is obvious by looking at the notarial certificate in the section where the notary signs. For example, if the notarial certificate reads, "Signed and sworn (or affirmed) before me...," it would be an Oath/Affirmation. If it reads, "I certify that this is a true and correct copy of a record...," it would be a Certified Copy. If it reads, "This record was acknowledged before me...," it would be an Acknowledgement. If it reads, "Signed before me...," it would be a Witness Signature.
However, a lawyer can definitively tell you which notarial act your document requires. Also, the provider of the document (the person or entity who created or provided the document) or the recipient of the document (the person or entity who will receive the document after it is signed and notarized ) should know which notarial act is required.
