The fourth requirement is that the worker must be eligible for medical treatment under the workers compensation law. Medical treatment must be provided to the injured worker for the injury for which he or she seeks compensation.
The fifth and final requirement is that the worker must file a claim for workers compensation benefits. A claim for workers compensation benefits includes filing a form, giving notice of the claim to the employer, providing proof of the injury, and providing proof of the medical treatment.
