And as stated in our official rules, you must comply with all the requirements and fill out the Claim Form and the Affidavit by placing your name on each paragraph, if we do not receive the properly signed documents from you by the due date, your prize will be forfeited, and if we don’t hear back from you in that time frame, we will select another winner. Once we have received the enclosed documents and completed the verification process, these funds will be forwarded via Direct Deposit or Certified Check.
