If you bought something on behalf of the PTA and need to be reimbursed OR you're a teacher member of the PTA at Warren and need to be reimbursed for classroom purchases.... please follow the steps below.

1- Open the file Reimbursement Form and either print the document OR go to the "File" menu and click "Make a copy"

2- Complete the highlighted areas

3- Paste a screen shot or copy of the receipt on the second page OR attach in the email/hard copy in step 4.

4- Email the document to warrencubspta@gmail.com with receipts included OR put hard copy of the document in the PTA mailbox.

5- Expect 3-4 weeks for reimbursement!
