Report on safety incident May 5, 2026 to NZTA:
Incident category: Derailment
Date and time of incident: 05/05/2026 0930 am.
Accident/incident description:
While moving the AL carriage out of the engineering workshop so repair work could be carried out, the points in front of the station were set incorrectly. TR 407 ended up with one set of her wheels on each side of the points before it was stopped.
Remedial actions taken:
On inspection we were able to get TR 407 to back up, which happened. We assessed what damage was done and how we would remedy the situation. The repairs were carried out and the line was reinstated for service again. The Operations Manager and Track Manager were informed of the situation.

Description of property: Points for our main line and sidling
Description of damage: One side of the points were forced across and needed to be realigned.
This has been done and they are all good for service again.
Operations Manager and Track Manager have been constantly kept up to date with what has happened.
NEW PROCEDURE FOR TWO-WAY RADIO COMMUNICATIONS
Effective May 12, 2026
Following two recent incidents -
Use of the main radio in the Portland 11 cab is to be discontinued.
Two hand-held radios will be used in the cab, with one only live, the other as standby,
and for use when shunting and running around.
Guards will also always have two radios, one as standby.
