Required Documents

• Photo identification (driver's license/state ID/passport) for you and your spouse (if filing a joint return)

• Social Security cards or Individual Taxpayer Identification Number documents for you, your spouse, and dependents

• Birth dates for you, spouse, and dependents

• A copy of last year's tax return

• Any tax documents received in the mail (W-2, 1098, 1099)

• Information for other income

• Proof of Educational-Related Expenses, Day-Care payments with Daycare's TIN/SSN, address, and phone number

• Proof of charitable donations
